Must-HaveProficient in OASIS and Home Care Home Base EMR, used within the last 6 months. Willingness to travel up to 75 miles from assigned care center. Comfortable driving to patient homes daily.
Description:
Provides and directs the provision of home nursing care according to physician's orders. Responsible for coordinating care based on agency policies and procedures. Competently applies the nursing process in patient care. Supervises and evaluates Home Health Aides and Licensed Practical Nurses. Must have reliable transportation, current driver's license, and agency-required liability insurance. Home visits and periodic office meetings are required. Average patient load is between 5-10 patients per day.
Each visit and activity holds different point values that contribute to weekly productivity points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation impacting billing and patient care will result in cancellation. There are no guaranteed hours with this contract. Clinicians must meet weekly productivity point requirements.
If patient census is low, clinicians may have the opportunity to float to nearby clinics to pick up hours.
