• one (1) year of registered nurse (rn) experience.

• bsn or master’s degree in nursing, nursing administration, nursing education or public health may substitute for the one (1) year experience.

• conducts comprehensive initial nursing assessments; determines program eligibility, needed services and payer source. Develops an individualized plan of care in consultation with the client’s physician and other appropriate agency personnel. Completes review of medications; assesses pain management and need for health care equipment/technology. Provides intermittent skilled nursing services, as ordered by the physician. Obtains specimens, as ordered, and submits to state lab utilizing appropriate laboratory services guidelines.
