Registered Nurse – Home Health

$1996.48/week –

Job Summary:

Provide comprehensive home health nursing care to patients requiring assistance in their residential environment. Coordinate and deliver personalized patient care in accordance with physician orders and agency protocols.

Responsibilities:

• Administer direct patient care during home visits, including wound care, assessments, and patient education.
• Coordinate with physicians and interdisciplinary teams to develop and implement care plans.
• Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses.
• Document clinical findings and patient interactions accurately using Home Care Home Base (HCHB) and Oasis as required.
• Maintain compliance with travel, documentation, and safety policies, including on-call responsibilities.
• Manage travel between patient homes within a 75-mile radius, ensuring timely and quality care delivery.

Qualifications:

• Hold a valid RN license in the state of practice with no pending restrictions.
• Have a minimum of 1 year of recent adult home health or hospice experience.
• Proficient in Oasis documentation and Home Care Home Base EMR within the last 6 months.
• Possess current CPR and BLS certifications.
• Have reliable transportation, a driver’s license, and appropriate auto insurance.
• Willing to travel up to 75 miles from the assigned clinic.
