Three Oaks Hospice is seeking a Registered Nurse to provide skilled nursing care to diverse patients in their homes, coordinating with the interdisciplinary team and families to ensure high-quality, compassionate care. You will implement and revise care plans, educate families on end-of-life care, and manage on-call duties.
The role requires an active RN license, CPR, and at least two years of clinical experience; hospice or home health experience is preferred.
