Primary Care Assistant

Position:

Primary Care Patient Care LPN

Collaborate with primary care providers to deliver safe, effective, patient-centered care, focusing on quality outcomes and supporting the clinical care team.

Assist in patient care activities, communicate effectively with patients and team members, and perform tasks within scope of practice independently with minimal oversight.

Graduate of an accredited practical nursing program, licensed as an LPN in the practice state, with preferred healthcare experience in a primary care setting.
