A growing pain management practice in Sacramento is hiring NPs and PAs to provide specialized pain and palliative care to geriatric residents across skilled nursing facilities in the area. You'll round on the same residents week after week, building individualized treatment plans, managing pain medications, and coordinating with facility staff and physicians. New positions open as additional facilities come on board, so this is a growth role alongside a growing practice.
New graduates genuinely welcome with formal onboarding; 5+ years of relevant RN experience also counts. Malpractice insurance with tail coverage included, which is uncommon and protects you from claims filed after you leave.
- Conduct comprehensive pain assessments including medical history review, physical exams, and identifying source and level of pain
- Develop individualized, evidence-based treatment plans for acute and chronic pain combining pharmacological, non-pharmacological, and interventional approaches
- Prescribe and manage pain medications, monitoring dosages per state and federal regulations
- Refer to and coordinate additional therapies including physical therapy, acupuncture, spinal blocks, and interventional techniques
- Educate patients and families on pain management strategies, coping mechanisms, and lifestyle modification
- Coordinate care with physicians, physical therapists, and facility providers to optimize outcomes
- Monitor treatment effectiveness through follow-up visits and adjust interventions as needed
- Maintain accurate, timely records in TEBRA and Point Click Care
- Build strong working relationships with skilled nursing facility leadership and staff
Required:
- Active, unrestricted California NP or PA license
- AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
- Fully onsite at skilled nursing facilities; cannot be done remotely or hybrid
- Able to travel between facilities in the Sacramento area
- Strong communication skills, compassion, and critical thinking
Preferred:
- Background in skilled nursing, long-term care, palliative care, or hospice
- Solid understanding of pain management pharmacology and psychosocial aspects of pain
- TEBRA or Point Click Care experience
- $60-$100/hour + productivity bonus | ~$115,200-$192,000/year
- Full-time | ~32 hours/week | Schedule varies by facility and caseload
- Onsite at Sacramento area skilled nursing facilities
- Malpractice insurance with tail coverage included
- Formal onboarding program
