The Nurse Manager of Case Management and Discharge Planning will report to an experienced senior nursing leader and receive support from hospital administration, physician partners, and interdisciplinary department leaders. The Nurse Manager of Case Management and Discharge Planning will oversee case management, utilization review, discharge planning, regulatory compliance, team development, and complex patient transitions while collaborating with friendly registered nurses, social workers, physicians, therapists, pharmacists, and a diverse team of healthcare professionals.
