California Wound Healing Medical Group is a physician-led mobile specialty practice providing advanced wound care services to patients in Skilled Nursing Facilities (SNFs) and other care settings throughout California.

We are seeking a Clinical Quality Assurance Licensed Vocational Nurse (LVN) to join our growing team in a field-focused hybrid role combining hands-on clinical support with in-office quality assurance responsibilities.

Reporting to the Quality Assurance Manager, you'll help maintain the highest standards of clinical quality, documentation, provider education, and patient care while supporting our providers and facility partners throughout Southern California.

About the Role

This position is headquartered in our Alhambra office and combines regular field assignments with in-office Clinical Quality Assurance responsibilities.

As a Field-Focused Clinical QA LVN, you'll travel to Skilled Nursing Facilities and other assigned locations to conduct skin sweeps and evaluations, support clinical quality initiatives, participate in provider and facility education, assist with provider onboarding and training, and help ensure our clinical and documentation standards are consistently implemented in the field.

When you are not scheduled for field assignments, you will work on-site at our Alhambra office as part of the Quality Assurance team, performing chart reviews, documentation audits, provider follow-up, clinical monitoring, and other QA responsibilities.

The frequency and location of field assignments will vary based on operational needs. Candidates should be comfortable transitioning between office and field responsibilities and with a schedule that may include multiple field days per week.

Most travel consists of local day trips throughout Southern California. Occasional overnight or air travel may be required for special projects, training, facility launches, or operational support.

This position may also participate in occasional weekend coverage of the company's G-tube support hotline based on scheduling and operational needs.

All approved business travel expenses are covered by the company, including mileage reimbursement, airfare, lodging, meals, rental vehicles, and other necessary approved travel expenses.

What You'll Do

  • Review provider documentation for accuracy, completeness, clinical consistency, and compliance with company and regulatory standards.
  • Conduct chart audits, identify documentation deficiencies, and coordinate necessary corrections and provider follow-up.
  • Monitor documentation timeliness, wound healing progress, and other clinical quality indicators.
  • Review and support biologic documentation and other specialized wound care documentation requirements.
  • Serve as a clinical and documentation resource for physicians, nurse practitioners, physician assistants, and other members of the care team.
  • Train and onboard providers on documentation standards, clinical workflows, quality expectations, and company protocols.
  • Conduct field-based quality assurance visits at Skilled Nursing Facilities and other assigned locations throughout Southern California.
  • Perform skin sweeps, skin evaluations, and other clinical support activities during field visits.
  • Participate in skills fairs, facility education, and clinical training initiatives for facility staff and providers.
  • Support provider onboarding and training in the field, including observation, education, and reinforcement of clinical and documentation standards.
