Home Health Registered Nurse

• Perform in-home skilled nursing visits
• Complete comprehensive assessments and ongoing evaluations
• Administer medications, wound care, injections, and other skilled procedures
• Update and follow individualized care plans
• Communicate with physicians and interdisciplinary team members
• Provide patient and caregiver education
• Document accurately and timely
• Travel within Rowland Heights and nearby areas
• Work independently with support from the office and clinical leadership

• Active California RN license (required)
• Home Health or acute care experience preferred
• Strong assessment and clinical judgment skills
• Reliable transportation and valid driver’s license
• Ability to work independently with strong time management
• Knowledge of OASIS (preferred but not required)

Demonstrates expertise in skilled nursing care, including comprehensive assessments, medication administration, and wound care, while effectively communicating with interdisciplinary teams and educating patients and caregivers. Possesses strong time management skills and the ability to work independently in home health settings.
