** Make a positive difference in the lives of our residents every dayYour Impact & Responsibilities As an LVN at City View Post-Acute, you will provide essential nursing care while helping ensure residents receive safe, compassionate, and individualized care.
* ** Clinical Care:
** Provide high-quality nursing care in accordance with physician orders, care plans, and facility protocols.
* ** Resident Monitoring:
** Observe residents, monitor changes in condition, and promptly communicate concerns to the appropriate healthcare professionals.
* ** Medication & Treatments:
** Administer medications and treatments safely and accurately.

* Collaboration:
** Work closely with nursing leadership, RNs, CNAs, rehabilitation staff, physicians, and other members of the interdisciplinary team.
* ** Documentation:
** Maintain accurate and timely clinical documentation and communicate resident progress effectively.
* ** Resident Advocacy:
** Build positive relationships with residents and families while promoting dignity, safety, and quality of life.
Qualifications
* ** Licensure:
** Current and valid
** California Licensed Vocational Nurse (LVN) license**.
* ** Communication:
** Strong written and verbal communication skills.
* ** Clinical
Skills:
** Ability to follow nursing standards, physician orders, and facility protocols.
* ** Teamwork:
** Ability to work effectively in a collaborative, team-oriented environment.

* Experience:
** Skilled nursing or post-acute experience is preferred, but
** new graduates are welcome to apply**.
* ** Attitude:
