Travel LPN - Home Health/Hospice
Location:
St. Louis City/County, Jefferson County, and St. Charles City/County
Weekly Hours Guaranteed: 32 Hours
Productivity Expectation: 25 points per week
Position OverviewWe are seeking an experienced and compassionate Travel Licensed Practical Nurse (LPN) to join our Home Health/Hospice team. This role involves providing high-quality patient care in home-based settings while supporting patients and families through education, treatment, and ongoing clinical interventions.

- Jefferson County
- St. Charles City
- St. Charles County
- Provide skilled nursing care to home health and hospice patients.
- Perform comprehensive patient assessments and monitor health status.
- Administer and manage wound care treatments.
- Provide medication administration and medication education to patients and caregivers.
- Deliver IV therapy and IV care as ordered.
- Educate patients and families on disease processes, treatment plans, and self-care strategies.
- Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care.
- Maintain accurate and timely clinical documentation in accordance with agency and regulatory guidelines.
- Current Licensed Practical Nurse (LPN) license in good standing.
- Previous Home Health and/or Hospice experience required.
- Strong clinical assessment and patient care skills.
- Experience with wound care, medication management, and patient education.
- Ability to work independently and manage an assigned territory.
- IV Certification preferred.
- Prior experience with IV therapy and infusion-related care.
- Excellent communication and patient education skills.
- Strong time management and organizational abilities.
