As an Advanced Practice Provider, you'll deliver high-quality, patient-centered care as part of a collaborative team-based care model. Working alongside physicians, nurses, medical assistants, care coordinators, therapists, and support staff, you'll help improve access to care, enhance patient outcomes, and strengthen the health of the communities we serve.

What You'll Do Evaluate, diagnose, and treat patients across the lifespan within the scope of licensure, credentialing, and organizational policies.

Obtain comprehensive patient histories, perform physical examinations, order and interpret diagnostic testing, and develop evidence-based treatment plans that support whole-person care.

Prescribe medications and therapies in accordance with state and federal regulations, credentialing privileges, and organizational guidelines.

Manage acute illnesses, chronic conditions, preventive care needs, and population health initiatives through a coordinated and patient-centered approach.

Collaborate with physicians and interdisciplinary team members to coordinate care, improve outcomes, and promote continuity throughout the patient care journey.

Provide patient and family education regarding diagnoses, treatment plans, medications, preventive health measures, and wellness goals.

Participate in care coordination activities, team huddles, quality improvement efforts, and practice initiatives designed to improve patient outcomes and patient satisfaction.

Maintain accurate, timely, and compliant documentation within the electronic medical record while meeting quality, regulatory, and organizational requirements.

Support organizational growth, patient access initiatives, and clinic performance goals through active engagement and collaboration with the care team.

What You'll Bring

Required Qualifications Current licensure as a Nurse Practitioner or Physician Assistant in the State of Missouri.

Strong clinical assessment, diagnostic, and treatment-planning skills.

Excellent communication, relationship-building, and patient education abilities.

Ability to work effectively within a team-based care environment.

Commitment to evidence-based practice, patient safety, and exceptional customer service.

Strong organizational skills and the ability to manage multiple priorities efficiently.

Preferred Qualities Primary care, family medicine, internal medicine, urgent care, or specialty clinic experience.

Experience practicing within a collaborative team-based care model.

Knowledge of population health and chronic disease management strategies.

Rural healthcare experience.

Interest in leadership, quality improvement, and patient experience initiatives.
