Sweetwater Care, a skilled nursing facilitator with ten facilites in Central Valley and Tulare area, is seeking an experienced Clinical Resource Nurse, licensed as an RN in California who will partner with our current clinical resources and clinical leaders for our facilities in the area to enhance an atmophere where excellent resident outcomes are the focus. The ideal candidate must possess at least four years of experience as a Director of Nursing for Skilled Nursing Care facilities, and a mimimum of two years as a Clinical Resource or like postion.

Any applicant should have a strong understanding of CMS and CDPH regulatory requirements, possess the capacity to lead staff and be willing to travel amongst the assigned facilities, as well as out of state, on a frequent basis.

The primary purpose of your job position is to steward a small ratio of facilities, and coach the Directors of Nursing in your region toward clinical, cultural, and financial excellence through intentional leadership. You will guide the teams to care for their residents and staff not only to meet the regulatory requirements but assist to refine processes that help the teams become the best care givers they can be.

This job will require constant presence in the facilities and will not be remote. Any canidate will need to live in close proximiy to the facilites they support.

A Clinical Resource is responsible for the oversight of clinical operations for assigned skilled nursing facilities. This role works collaboratively with facility clinical leadership teams, and other resource services team members to ensure the delivery of quality care and regulatory compliance. The Clinical Resource acts as the primary resource for training, monitoring and coaching facility Directors of Nursing and Nursing Department managers.

This role also assists such facilities during annual and complaint surveys as well as assisting with preparation and implementation of plans of correction. This role will work collaboratively with our outside consulting firms that are onsite regularly and Sweetwater’s Director of Clinical Services. This position will require extensive travel (75% of time) to our four facilities in Tulare, Visalia, and Porterville.

Qualifications

Education:
  • Graduate of accredited school of nursing.
Licenses/Certification:
  • Valid RN license in the state(s) employed.
  • Valid CPR certification.
  • Infection Preventionist certification (optional)
  • Valid driver’s license.
Experience:
  • 4 years successful experience as a Director of Nursing or equivalent position in long-term care facility preferred.
  • 2 years expereince as a clinical resource
  • Multi-site experience preferred.
Essential Functions
  • Conducts on-site reviews of clinical operations, medical records, and staff performance.
  • Review’s facility clinical activities and collaborates on root-cause analysis and action plans.
  • Educates facility staff in clinical and regulatory issues, as well as facility policies.
  • Provides guidance and assistance to Directors of Nursing. Acts as a mentor for professional growth.
  • Establishes and maintains a peer networking system for Directors of Nursing.
  • Identifies quality of care issues and takes appropriate actions.
  • Validates the clinical competencies and effectiveness of the DON and SDC.
  • Is resourceful in provision of clinical staff training to achieve successful performance.
  • Participates with the orientation of the facility DON and SDC.
  • When in a facility, promotes an environment where residents’ rights are protected, and residents are free from abuse and neglect.
  • Participates in the company’s employee recognition programs.
  • Quantifies data for tracking and trending, supporting a strong Quality Assurance and Performance Improvement program.
