Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a continuum of homelessness prevention, street outreach, employment preparation and placement, individualized case management, supportive services, interim housing, and permanent supportive housing.
About the JobThe Case Manager helps clients break the cycle of homelessness by using a whatever it takes approach, supporting clients through all phases of homelessness (on the street, interim housing, transitional housing, and permanent housing). They assist clients in accessing and maintaining housing, healthcare, social services, interpersonal support systems, and navigating a coordinated entry system (CES).
Position
Responsibilities include:
- Conduct intake and individualized needs assessment for all participants; collaborate to develop an Individualized Service Plan (ISP) addressing housing goals, income goals, and other personal goals (medical, mental health, substance use, financial resources, vocational, and social support needs).
- Provide referrals, linkages, information, and support to resources that help participants achieve ISP goals.
- Complete program-specific assessments (e.g., VI-SPDAT).
- Evaluate strengths and challenges to address short- and long-term goals; conduct a 90-day review with the participant after initial intake.
- Maintain documentation standards as required by the Program and PATH policies, including timely case file notes and a complete, accurate, and current participant file with all required documents.

Preferred Qualifications
- Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
Minimum Qualifications
- All levels of education and experience are welcome.
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able to obtain CPR/First Aid training
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
Driving is an essential function of this position. To meet employment requirements, all candidates must:
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- Ability to qualify for PATH's insurance coverage
