LHH Recruitment Solutions is searching for a full‑time Case Manager on a contract basis for a growing client in Ventura, CA. Our client is seeking individuals that are highly dynamic and adaptable, empathetic, and committed to addressing the unique challenges faced by their clientele.
This is an onsite position and the candidate needs to live in or near Ventura, CA.
Responsibilities:
Conduct comprehensive assessments to determine the needs, strengths, and barriers of individuals or families seeking assistance.
Interview clients, review their history, and evaluate their immediate and long‑term needs.
Collaborate with clients to develop individualized case plans that outline short‑term and long‑term goals.
Identify and connect clients with appropriate resources.
Coordinate with other service providers and community organizations to ensure clients receive the necessary support.
Serve as an advocate for clients, ensuring their rights are protected, and help them navigate complex systems.
Provide immediate assistance and support during crises.
Offer guidance and training to enhance clients’ independent living skills.
Regularly check in with clients to monitor progress, assess ongoing needs, and adjust the case plan as necessary.
Maintain accurate and confidential case records, including client assessments, case plans, progress notes, and other required documentation.
Qualifications:
- High school diploma/GED, required.
- Bachelor’s Degree with 2+ years of experience in a related field, required.
- Master’s Degree with 1+ year of experience in healthcare, required.
- Proficiency with Microsoft Office, required.
- Reliable transportation, required.
- Valid driver’s license, required.
Compensation/Benefits: $24‑$25/hr
Job Type: Contract to hire
Benefit offerings include medical, dental, vision, life insurance, short‑term disability, additional voluntary benefits, EAP program, commuter benefits, and a 401K plan. Pay is $24 to $25 per hour. Paid leave may include Paid Sick Leave as required by law, any other paid leave required by Federal, State, or local law, and Holiday pay upon meeting eligibility criteria.
