Location: California

Wellness Center Operations & Collaboration Support day-to-day operations of assigned Wellness Center sites in alignment with SBWP policies and procedures.

Collaborate with school administrators, teachers, and staff to support student wellbeing.

Participate in enrollment, outreach, and engagement activities.

Monitor Wellness Center supply needs.

Student & Family Engagement Conduct outreach to students and families upon referral or enrollment.

Build trusting, strengths-based relationships using trauma-informed practices.

Develop goals collaboratively with students and families.

Provide 1:1 mentoring/coaching and crisis-responsive support.

Conduct home visits in accordance with SBWP safety protocols.

Screening, Assessment & Resource Navigation Administer ACEs (PEARLS) and SDOH Screenings.

Identify needs and strengths and coordinate appropriate supports.

Connect students and families to school-based and community resources.

Support closed-loop referrals and follow-up.

Documentation Maintain timely, accurate case notes in NCCC and other tracking documents.

Complete a daily communication log for assigned Wellness Center sites.

Track Empowerment Group attendances/rosters.

Upload required documentation within required time frames.

Assist with data collection and reporting.

Communicate progress with referral sources as appropriate.

Health Promotion & Programming Support wellness education, empowerment groups, and school-wide health promotion activities.

Facilitate weekly assigned Empowerment Groups using evidence-based curriculum.

Other Responsibilities Maintain knowledge of community resources.

Uphold confidentiality and mandated reporting requirements.

Participate in supervision and professional development.

Perform other duties as assigned.

Qualifications:

Required4-5 years experience working with children, adolescents, or young adults.

A.A. degree in related field

Strong communication and organizational skills.

Ability to work independently and as part of a team.

Computer proficiency and ability to learn NCCC.Valid CA Driver’s License and reliable transportation.

Preferred Experience with trauma-informed care, ACEs/SDOH screening, or care coordination.

Knowledge of school-based systems and community resources.
