Pay or shift range: $35.16 USD to $50.23 USD. The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job‑related reasons.

Provide Compassionate Support When Patients Need It Most

At Valley Presbyterian Hospital, our Social Services team plays a vital role in helping patients and families navigate complex medical, emotional, and social challenges. We are seeking a compassionate and resourceful Medical Social Worker to provide psychosocial assessments, counseling, discharge planning support, and community resource coordination for patients across the continuum of care.

• Conduct psychosocial assessments to evaluate patients’ social, emotional, cultural, financial, and family needs
• Provide counseling and emotional support to patients and families coping with acute, chronic, or terminal illnesses
• Assist patients and families in understanding and following medical recommendations
• Coordinate referrals to community agencies and support services
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams to support optimal patient outcomes
• Support discharge planning for patients with complex psychosocial needs
• Respond to referrals, emergency requests, and patient care needs in a timely manner
• Maintain accurate, thorough, and compliant documentation
• Participate in quality improvement initiatives, staff meetings, committee work, and departmental projects
• Assist with orientation and training of new staff as needed
• Provide on-call coverage and weekend/holiday support based on departmental needs

• Strong clinical assessment and counseling skills
• Ability to work independently and manage a dynamic caseload
• Excellent communication and interpersonal skills
• Strong organizational and documentation abilities
• Ability to build collaborative relationships with patients, families, physicians, and staff
• Commitment to ethical practice and patient advocacy
• Bilingual skills highly valued

• Minimum two (2) years of hospital social work experience preferred

• Master’s Degree in Social Work (MSW) required

• Management of Assaultive Behavior (CPI or equivalent) certification within 30 days of hire
• LA City Fire Card certification within 30 days of hire
• Available for a minimum of two shifts per month
• Availability for weekends, holidays, and on‑call assignments as scheduled
• Holiday coverage to include one major and two minor holidays annually
