Your Role at Sana LakeSana Lake is expanding rapidly and is seeking a Clinical Chart Auditor who is passionate about helping individuals and families recover!
We would love to learn more about you if you:
- Associate degree in a relevant field, preferred
- Minimum of one year of experience in behavioral health or substance use disorder
- Experience with an electronic health record system; preferred
- Familiarity with ASAM Criteria, Missouri Department of Mental Health requirements, Joint Commission standards, HIPAA, 42 CFR Part 2, and payer documentation expectations
- Conduct routine and focused audits of client records across applicable levels of care, including detoxification, residential, PHP, IOP, outpatient, and recovery-support services
- Review documentation completed by counselors, therapists, case managers, nurses, physicians, nurse practitioners, physician assistants, and other members of the treatment team
- Evaluate records for completeness, accuracy, consistency, timeliness, appropriate authentication, and compliance with organizational policies
- Confirm that documentation accurately reflects the services provided, the client’s clinical condition, progress toward treatment goals, and ongoing need for the current level of care.
- Review admission assessments, biopsychosocial assessments, medical evaluations, treatment plans, progress notes, group notes, individual and family therapy notes, case-management documentation, medication records, continued-stay reviews, transition plans, and discharge summaries
