From 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health merged to form a new, unified not‑for‑profit community health organisation, Omnia Community Health. This decision follows an extensive exploration process that confirms the merger improves access to safe, high‑quality and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.
The new organisation brings together the deep community connections, shared values and specialist expertise of both entities. Services span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding communities.

About the RoleThe Men’s Case Management Worker provides tailored, individualised support to men who use family violence, with the primary goal of increasing victim survivor safety and promoting perpetrator accountability. This role involves conducting comprehensive risk assessments,developing and implementing case plans,in collaboration witha
Partner Safety Worker,and actively engaging clients through assertive outreach and motivational approaches which supports the safety of women and children. The worker coordinates referrals to specialist services such as mental health, alcohol and other drugs (AOD), housing, and Men’s Behaviour Change Programs (MBCPs), whilemaintainingstrong interagency collaboration and compliance with family violence information-sharing legislation.
The position requires challenging violent behaviours and attitudes, supporting perpetrators to take responsibility for their actions, and ensuring family safety contact processes are integrated into service delivery.
Additional responsibilities include managing brokerage funding to reduce risk, monitoring progress, andmaintainingaccuratedocumentation and reporting. The role demands cultural competence, adherence to gender-informed practice, and commitment to professional standards, including regular supervision and reflective practice.
- Qualifications in social work, psychology, counselling, or a related discipline.
- Most hold current National Criminal History Check, NDIS Worker Screening check and valid Working with Children Check and hold valid Australian working rights.
- Must hold current
Victorian
Driver’sLicence.
