Responsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities and demonstrates the knowledge and skills necessary to provide exceptional care for the population served.

• Collect patient health data.
• Analyze the assessment data in determining nursing diagnoses/problem focus.
• Identify expected outcomes/goals individualized to the patient.
• Develop a plan of care that prescribes interventions to attain expected outcomes.
• Implement the interventions identified in the plan of care.
• Evaluate the patient’s progress toward attainment of the outcomes.
• Systemically evaluate the quality and effectiveness of nursing practice.
• Collaborate with the patient, significant others, and healthcare providers in providing patient care.
• Utilize research/evidence‑based findings in practice when appropriate.
• Consider factors related to safety, effectiveness, and cost in planning and delivering patient care.

• Valid driver’s license; employee must be insurable by the Avera Health automobile liability insurance carrier upon hire.
• Registered Nurse (RN) license in the state of practice; active license required upon hire.

• Minimum two (2) years of related experience and/or training or an equivalent combination of education and experience.
