Registered Nurse (RN) - Visiting Nurse

Role Overview:
Responsible for providing nursing care in patients' homes or facilities by utilizing the nursing process according to ANA standards. The Nurse assesses patient health data, develops individualized care plans, implements interventions, and evaluates patient progress. They collaborate with patients, families, and healthcare providers, using evidence-based practices to ensure safe, effective, and cost-efficient care.

• Collect and analyze patient health data
• Develop and implement personalized care plans
• Evaluate patient outcomes and adjust care as needed
• Collaborate with healthcare team and patients
• Apply research findings in practice
• Prioritize safety, effectiveness, and cost considerations

• Valid driver's license and insurable by the employer
• Active RN license in the practice state
• Effective communication, hearing, and visual acuity
• Preferred: minimum two years related experience
