Registered Nurse (RN) - Long Term Care

Role Overview: Provides comprehensive nursing care within a multidisciplinary team, focusing on physical, emotional, psychological, and spiritual support for residents and their families.

• Develops and implements personalized nursing care plans based on medical histories and resident assessments.
• Coordinates with physicians and healthcare providers to review prescriptions and communicate care plans.
• Educates residents and families on health conditions, medications, and self-care practices.
• Monitors and evaluates resident progress, adjusting care plans as needed to ensure optimal outcomes.
• Maintains a safe environment by adhering to infection control, medication, and safety protocols.
• Ensures proper functioning of care equipment and maintains adequate supplies.
• Handles admissions, discharges, and transfers, and supervises nursing assistants and staff workflow.

• Active Registered Nurse (RN) license in the practice state.
• Visual and communication skills necessary for effective patient care.
• Preferred 1-3 years of clinical or long-term care experience.
