Child and Adolescent Care Coordinator (case manager)
Qualifications:
Bachelor degree in psychology, social work, sociology, or other health care related field required
Preferred Experience:
Previous work with children and adolescents
Duties and Responsibilities:
Provide mental health case management services to children and adolescents.
Complete needs assessments and develop caseplans. Works closely with the family unit and community programs for the welfare of the child/adolescent.
Works to keepindividuals stabilized in the community settings and assists in securing psychiatric hospital placement as needed. Works closely with the C&A In Home team to provide needed case management services as well as functions as the liasion between In Home and Ouptatient services.
Assist with crisis situations as needed.
