ADRC Coordinator

The ADRC Coordinator is responsible for the completion of comprehensive assessments to evaluate need and determine supports available to callers as well as follow-up to ensure assistance is obtained. Assures accuracy of the agency's resource database. The ADRC Coordinator is responsible for coordinating public education and outreach activities throughout the service area and assisting with other special projects. Assists in development and implementation of special projects and programs as needed.

The ADRC Coordinator receives general supervision from the Director of AAA. Supervisor sets the overall objectives and employee and supervisor, in consultation, develop the deadlines, projects, and work to be done; employee is responsible for planning and carrying out the assignment; and work is reviewed only from an overall standpoint.

Responsibilities and duties include:

  • Develops and implements public education and communication strategies to increase public awareness and utilization of the AAA's various programs and services.
  • Networks with community groups to strengthen and expand relationships and program recognition at the local, regional and State level.
  • Provides community education sessions as needed.
  • Performs intake duties for callers and walk-ins as needed.
  • Provides information and referrals by establishing contact with the consumer, assessing the individual's needs, identifying resources to meet those needs, providing a referral to the identified resources and following up to ensure that the consumer's needs have been met.
  • Assists with completion of applications for assistance, follows up with service agencies on the delivery of services and provides short term case management.
  • Assists with Part D annual enrollment when necessary.
  • Assures the accuracy of the resource database.
  • Collects caller demographic information.
  • Assures all client records are maintained accurately by following network standards for Peer Place.
  • Completes reporting requirements, including but not limited to, Alabama Department of Senior Services reporting.
  • Trains ADRC staff on intake procedures, record keeping and appropriate client tracking.
  • Visits with local agencies, doctors and businesses throughout the region to share information on the Area Agency on Aging and distribute brochures and other appropriate information.
  • Other duties as assigned

Knowledge, skills, and abilities required include:

  • Ability to comprehend the goals, objectives, and regulation of the various aging programs.
  • Ability to analyze program guidelines and make decisions.
  • Knowledge of Medicare and Medicaid Programs.
  • Knowledge of patient rights under the Health Insurance Portability and Accountability Act (HIPAA).
  • Ability to establish and maintain effective working relationships with event sponsors, state agencies, clients and their families.
  • Excellent communication skills, both oral and written.
  • Good record keeping, time management, and organization skills.
  • Ability to operate a computer with knowledge of Windows, Excel and Word.
  • Ability to develop and manage multiple projects.
  • Bachelor's Degree from an accredited four-year college or university in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or a related field with a minimum of four (4) years of work experience in home health or Aging programs.
  • Must obtain the following certifications within one year of hire:
    • Inform USA's Community Resources Specialist
    • Person Centered Training (PCT)
    • SHIP
  • Must be an Alabama Medicaid Certified Case Manager or obtain certification within 90 days.

Physical requirements include:

The work is sedentary. Typically, the employee may sit comfortably to do the work. However, there may be some walking; standing; bending; carrying of light items such as papers, books, small parts; driving an automobile, etc. This position requires regional travel to client locations on a regular basis.
