Position: Registered Nurse// Hospice & Senior Home Health// Full Time & Part Time Shifts
Job Description
We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality hospice and home health care to senior patients. This role focuses on patient comfort, dignity, and individualized care while working closely with families, caregivers, and interdisciplinary care teams.
  • Provide skilled nursing care to hospice and home health patients in their homes or senior living environments
  • Assess patient conditions and develop, implement, and evaluate individualized care plans
  • Administer medications, treatments, and wound care as ordered by physicians
  • Monitor patient symptoms, pain levels, and overall health status
  • Educate patients and families on care plans, medications, and symptom management
  • Collaborate with physicians, social workers, aides, and other healthcare professionals
  • Maintain accurate and timely patient documentation and reports
  • Provide emotional support and compassionate care to patients and families
  • Follow all state, federal, and company policies and healthcare regulations
  • Requirements
  • Active Registered Nurse (RN) license
  • Strong assessment, clinical, and communication skills
  • Compassionate, patient-centered approach to care
  • Ability to work independently in home-based settings
  • Valid driver's license and reliable transportation
  • CPR/BLS certification
  • Preferred Qualifications:
    • Hospice or palliative care experience
    • Experience working with geriatric patients
    • Knowledge of Medicare regulations and documentation standards
    Work Environment &

    Physical Requirements:

    • Travel to patient homes and senior residences
    • Ability to lift, reposition, or assist patients as needed
    • Emotional resilience and strong interpersonal skills
