Position Overview
Behavioral Health Unit – SSM Health St. Mary's Hospital, Madison, WI.
Job SummaryAssists in performing a variety of nursing care services for patients.
Job Responsibilities- Receives patient care assignment from the registered nurse (RN) in charge and initiates delegated tasks based on demonstrated competence in nursing activities for specific patient age groups.
- Collects and documents data for admission assessment and reassessment at scheduled times, providing appropriate data to the licensed nurse for analysis and validation of patient responses to care or treatment.
- Relays information via charting and communicates to team members.
- Informs the licensed nurse of any changes in patient condition/needs, patient complaints, and educational needs of patients or significant others.
- Answers patient calls for help, measures vital signs, monitors behavior and physical condition, and assists with bathing, ambulating, feeding, skin care, personal and oral hygiene, bed occupancy, dressing changes, enemas, ostomy care, toileting, turning, cough and deep breathing, catheter care, specimen collection, and intake/output recording.
- Prepares patients for discharge and accompanies them upon dismissal as indicated.
- Stocks and maintains supplies and equipment per patient care needs and cost containment guidelines.
- Assists with delegated clerical duties such as answering telephones, relaying messages, transcribing physician orders, and organizing the patient’s medical record.
- Orients new patients and significant others to the nursing unit.
- Operates equipment mechanically and manually as recommended, recognizing common mechanical problems and initiating corrective action.
- May perform point‑of‑care testing in accordance with policies and procedures.
- Maintains a constant state of alertness and works safely.
- Performs other duties as assigned.
- Must hold an active Wisconsin Certified Nursing Assistant (CNA) certification; or the equivalent certification required by state law for the location of assignment.
- No prior experience required; training will be provided.
- Constant standing and walking; frequent use of hearing, speech, vision, reaching, and keyboard data entry.
- Frequent lifting/carrying/pushing/pulling of objects weighing 0–25 lbs; occasional lifting of 25–50 lbs and moving patients.
- Occasional bending, stooping, crawling, kneeling, sitting, squatting, and repetitive hand/arm movements.
- Occasional use of vision for distance judgment and spatial relationships; occasional use of smell for odor detection; occasional driving.
Full‑time; scheduled 36 hours per week on rotating evening/night shifts (3 pm–11:30 pm / 11 pm–7:30 am), rotating weekend and holiday duties every other weekend/holiday.
