Northgate Health & Rehabilitation is seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our clinical leadership team. If you have a strong understanding of the MDS process, Medicare and Medicaid reimbursement, care planning, and clinical documentation, this is an excellent opportunity to make a meaningful impact while working alongside a supportive interdisciplinary team.

As our MDS Coordinator, you will play a vital role in ensuring accurate resident assessments, maximizing reimbursement, maintaining regulatory compliance, and supporting exceptional resident outcomes.

Responsibilities
  • Coordinate and complete the Resident Assessment Instrument (RAI) process, including MDS assessments and Care Area Assessments (CAAs)
  • Ensure timely and accurate completion and submission of all MDS assessments
  • Develop and coordinate comprehensive interdisciplinary care plans
  • Collaborate with nursing, therapy, dietary, social services, and physicians to ensure accurate clinical documentation
  • Monitor Medicare and Medicaid documentation to support appropriate reimbursement
  • Maintain compliance with CMS regulations and state guidelines
  • Participate in Medicare meetings, quality improvement initiatives, and clinical audits
  • Educate and support nursing staff regarding documentation requirements and regulatory standards
  • Assist with survey preparation and regulatory compliance activities
Qualifications
  • Current Alabama RN or LPN license in good standing
  • Previous MDS Coordinator experience preferred
  • Strong knowledge of RAI process, PDPM, Medicare, Medicaid, and CMS regulations
  • Experience with skilled nursing or long-term care preferred
  • Excellent assessment, documentation, organizational, and communication skills
  • Ability to manage multiple priorities while meeting regulatory deadlines
  • Experience with electronic medical records is preferred
