Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to deliver expert nursing care in patients’ homes or other living settings. You will perform assessments, implement plans of care, and coordinate with an interdisciplinary team while educating families and providing emotional support.
Current RN license and CPR certification required;
Hospice or home health experience preferred. This is a full-time role with benefits, travel, and on-call responsibilities.
