Responsibilities

This position is responsible for providing comprehensive, long‑term, structured, complex case management services for an assigned caseload of clients participating in residential treatment. The role assists the MAS nurse with medication administration, documentation, and reporting medication administration errors. It also creates and maintains client medication files, evaluates client progress, and conducts mentoring and case management sessions with clients and/or staff.

  • Meets regularly with clients to discuss and evaluate their progress; prepares accurate, up‑to‑date records.
  • Outsources clients to additional counseling resources if needed; maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
  • Updates nurse weekly on all new admissions into the DWL/NDP programs; informs nurse of any hospital admissions/discharges or medication changes; assists clients with medication refills and medical appointments.
  • Coordinates and/or facilitates life management, addiction‑related, behavior modification, etc. classes to assist clients.
Physical Requirements and Working Conditions

Work may involve driving to off‑site meetings where there may be physical discomforts associated with changes in weather or with noise, dust, dirt, and the like. Work is also performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or noise, dust, dirt, and the like.

Qualifications
  • Bachelor's degree or greater from an accredited college or university in Psychology, Social Work, Behavioral Science, or a related field.
  • Three years progressively responsible experience providing direct case management services, preferably in a substance abuse treatment related environment, OR
  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Licenses and Certifications
  • Valid Driver's License.
  • Must have the ability to become MAC certified within 30 days of employment.
