Oversee their program in Modesto/Stockton market
Monday through Friday daytime schedule with flexible hours
Geriatric and post-acute patients under Medicare and Medicaid programs

Experienced IM, FM, or geriatric-trained physicians with 3+ years of experience are preferred
2-3 day home visits (geographically grouped ) 1-2 days clinic and one day telehealth
