Garden City Health Care Center, a skilled nursing facility in Modesto, CA, is seeking a Certified Occupational Therapy Assistant (COTA) to join our dedicated rehabilitation team. Key Responsibilities

• Provide direct therapy services under the supervision of a licensed Occupational Therapist
• Implement and monitor individualized treatment plans
• Assist residents with therapeutic exercises and ADL training
• Document therapy sessions and progress accurately and in a timely manner
• Communicate effectively with residents, families, and the interdisciplinary care team
• Support discharge planning and caregiver education
• Ensure adherence to safety, regulatory, and facility standards

• Active California COTA license (required)
• Current CPR certification (required)
• Experience in a skilled nursing or post-acute care setting preferred
• Strong interpersonal and communication skills
• Commitment to providing compassionate, resident-centered care

• Starting wage of $42/hour
