The All-Inclusive Care for the Elderly (PACE) program supports seniors in receiving the care they need while living in their communities.

• Who qualifies for PACE? Individuals aged 55+ with Medicare or Medicaid (or both) who require a nursing home level of care but can safely live in the community with PACE support.
• How does PACE work? The program covers all necessary services, including provider visits, medications, home care, transportation, hospitalizations, and nursing home stays when required.
