• Evaluate patients and develop individualized physical therapy treatment plans that advance recovery and independence
• Provide skilled therapy interventions to improve mobility, strength, balance, and functional outcomes
• Document evaluations, treatments, and patient progress accurately and in a timely way
• Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes
• Ensure compliance with facility policies and state regulations while advocating for patient-centered care

• Active Physical Therapist license in the state of California (required)
• Skilled Nursing Facility experience preferred but not required
• Strong clinical reasoning, communication, and documentation skills
• Patient-focused and outcome-driven approach to care

• Proficient in patient evaluation and treatment planning
• Patient-centered care and clinical autonomy
• Accurate documentation and familiarity with clinical record-keeping
• Strong collaboration and communication with interdisciplinary teams
• Understanding of SNF workflows and regulatory compliance
