Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members. The Practitioner will float between various clinics as far north as Stockton and south in Merced. This is a full-time opportunity, scheduled Monday through Friday from 8 am - 5 pm. The provider will work on-site at the clinic, performing an average of 8-10 scheduled exams per day.
No on-call, nights, weekends, or walk-ins.

Position Summary:
Qualified Nurse Practitioners and Physician Assistants interested in this position will have the opportunity to interact with members in a one-on-one setting, supporting their comprehensive care team. Providers performing HRAs are not required to treat or prescribe.
Nurse Practitioner Qualifications and Experience Requirements:
- Masters Degree OR commensurate experience and satisfactory completion of NP license
- Active State NP license
- Active National Board certification
- Current BLS certification
- 12 months of experience as a paid advanced clinical provider is preferred
