• Shift: Day Shift
• Schedule:
  
  8:00 AM – 4:30 PM

• Active CA Registered Nurse (RN) license
• Current CPR or Basic Life Support (BLS) certification from an approved provider
• Previous Home Health and/or Care Management experience
• Valid driver's license
• Must be at least 21 years of age
• Current automobile insurance
• Reliable personal vehicle for patient visits
• Excellent communication, organizational, and care coordination skills
