Hospice services are delivered wherever patients call home, including private residences, hospitals, nursing facilities, and hospice centers.

Patients and families receive nursing care, symptom management, physician services, counseling, spiritual support, therapy, dietary services, medical equipment and supplies, volunteer support, and bereavement care. With locations across multiple states, Bristol Hospice partners with families, caregivers, and healthcare professionals to provide trusted, compassionate care in the communities it serves.

The RN Case Manager is a full-time, on-site role based in Modesto, CA, responsible for coordinating and delivering hospice nursing care to patients and families. This role assesses patient needs, develops and updates individualized care plans, and collaborates with physicians, interdisciplinary team members, and caregivers to ensure appropriate clinical interventions and symptom management. Daily responsibilities include regular patient visits, medication and treatment administration, monitoring changes in condition, and providing education and support to patients and families about disease processes, hospice services, and end-of-life care.

The RN Case Manager facilitates communication among the care team, participates in care conferences, and ensures timely documentation that meets regulatory and organizational standards. The role also supports transitions of care, advocates for patient preferences, and promotes safety, comfort, and dignity in all care settings.

• Clinical nursing skills in hospice or related settings, including strong proficiency in Nursing and fundamental knowledge of Medicine appropriate to patient care.
• Experience in Case Management, coordinating interdisciplinary services and resources to support individualized care plans for patients and families.
• Skills in Discharge Planning, ensuring safe transitions between care settings and effective communication with families, caregivers, and healthcare partners.
• Exposure to Utilization Management, with the ability to align care delivery with regulatory requirements and organizational policies while supporting quality outcomes.
• Current Registered Nurse (RN) license in the state of California; hospice, palliative, or home health experience strongly preferred.
• Strong assessment, clinical judgment, and documentation skills, with the ability to manage a caseload and prioritize multiple needs.
• Effective communication and collaboration skills, including the ability to work with diverse patients, families, and interdisciplinary team members.
• Valid driver’s license, reliable transportation, and ability to travel locally to patient homes and care facilities as needed.
