Brief Description

36-40 hours per week (Full-Time). The schedule includes weekday shifts, two evening shifts, and a four‑hour shift on Saturdays each week. Hours include Tuesdays and Thursdays 10 am‑7 pm, and Saturdays 8 am‑12 pm.

Case Managers at the Family Resource Center provide education, group facilitation, and supportive services to families with children ages 0–5, focusing on early learning, child development, and school readiness. They also provide differential response services for children at risk for abuse and neglect, and after‑care services for children ages 0–17 and their families. Case management involves assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet family needs through effective communication and connection to resources that promote overall well‑being.

Services are delivered in Family Resource Centers, client homes, and community settings.

Qualifications
  • Case Manager I – High school diploma or GED required. Bachelor’s degree in Counseling, Social Work, or related field preferred.
  • Case Manager II – Bachelor’s degree in counseling, social work, or related field required. Equivalent experience accepted.
  • Knowledge of community resources preferred.
  • Experience facilitating group sessions, including parent cafés and educational classes for adults and children preferred.
  • Knowledge of child development framework, including Adverse Childhood Experiences (ACEs) and stages questionnaires (ASQ), the Five Protective Factors Survey (PFS) assessment preferred.
  • Bilingual required.
  • Must be 21 years old or over.
  • A valid California driver’s license is required for staff who have regular driving duties, and the driving record must meet position requirements and be approved by the agency’s insurance company.

Responsibilities
  • Provide access, advocacy, resources, and linkage to clients.
  • Attend community events and provide outreach services, including identifying areas that need strengthening and setting and reaching targeted goals.
  • Administer program measurement tools and surveys to track and report program data and outcomes.
  • Interact productively within a multi‑disciplinary team and other community organizations.
  • Maintain record and tracking systems that meet standards of the agency, The Joint Commission, and contract.
  • Prevent and control infection.
  • Maintain a culture of quality and safety.
  • Use de‑escalation skills and techniques for crisis prevention.
  • Properly administer the C‑SSRS to help prevent suicide.
Other
  • Attend all required meetings and training.
  • Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  • Report any client imminent danger to self or others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  • Other duties as assigned.

Base Pay: $21.00–$28.38 per hour
