POSITION SUMMARY

Diverts youth ages 18-24 who are currently housed and experiencing different housing insecurities (i.e. high housing costs, relational issues, overcrowding, etc.). Works to keep youth housed through crisis response and intervention, negotiation or mediation, service linkages, education and employment services.

DUTIES
  • Conducts assessments of new clients and complete Health plan authorization referral forms.
  • Develops individual, youth driven plan that addresses barriers to success and supports housing stability and financial self-sufficiency.
  • Diverts youth from homelessness. This could include negotiations/mediation to remain in current living situation, linking/scheduling with appropriate resources and services including emergency shelter, temporary housing, permanent housing or Rapid Re-Housing assistance.
  • Provides and/or coordinates intensive case management services to all clients placed in programs including: counseling, advocacy, transportation; and assists youth in meeting basic needs of food, clothing and shelter.
  • Develops and assists clients with implementation of Individual Living Plans and/or Housing Care Plans.
  • Assists clients in accessing services that include progressive engagement, development of a youth driven plan, elimination of barriers and linkage to a variety of supportive services including mental health, substance use, education and employment.
  • Assists clients with developing independent living and employment skills. This will include teaching, facilitating and/ or arranging for classes or groups and activities based on current clients’ needs. Coaching clients in the following, but not limited to: skills associated with obtaining and keeping jobs, social interaction, conflict resolution, money management, personal hygiene, time management and organization.
  • Provides guidance to clients in reaching their goals and monitors success toward meeting them.
  • Meets regularly with clients in the field, at homes, schools, job sites, established CHS sites, and other appropriate sites.
  • Transports clients in Agency owned vehicles as needed and approved.
  • Interacts productively within a multi-disciplinary team to provide holistic case management services to an identified group of clients.
  • Tracks data and outcomes, provides reports and participates in data management to meet agency and funders requirements.
  • Completes follow-up duties with assigned clients.
  • Maintains daily communication with service delivery team and submits all required written documentation on a daily basis.
  • Maintains client file by daily charting of progress of clients toward achievement of established goals.
  • Complies with all case management plan development, documentation and record keeping requirements.
  • Provide direction and training to staff in implementing the support or treatment plan.
  • Completes accurately and timely related paperwork, progress notes, client data and file management. Responsible for monthly, quarterly and annual reports.
  • Maintains professional boundaries in providing crisis intervention and support services to client. Must apply positive behavior modification techniques, effective de-escalation and problem-solving skills.
  • Participates in weekly case review that includes development of independent living plans and family reunification when appropriate.
  • Performs other related duties as they are assigned.
