Position: Community Work Incentive Coordinator

Community Work Incentive Coordinator

We are looking for a Community Work Incentive Coordinator to support individuals with disabilities and their families by explaining available benefit options and employment-related incentive programs in Modesto, California. This long-term contract position is ideal for someone who can combine benefits coordination, community engagement, and case documentation to help clients make informed decisions about work and coverage. The role requires close collaboration with public agencies, nonprofit partners, and service providers while delivering clear guidance, outreach, and educational support.

Responsibilities include:

  • Guiding individuals with disabilities and their families through federal and state work incentive programs, helping them understand how employment may affect public benefits and health coverage.
  • Meeting with clients to complete intake interviews, assess service needs, maintain organized case records, and document research and follow-up activities accurately.
  • Building relationships with government offices, nonprofit organizations, employment networks, and other community partners that support people with disabilities.
  • Leading outreach activities, informational sessions, and community presentations to increase awareness of available services and work incentive opportunities.
  • Evaluating employer-sponsored health coverage and discussing alternative benefit options that may be available to clients based on their circumstances.
  • Providing individualized work incentive planning that supports beneficiaries as they pursue employment goals and navigate benefit changes.
  • Creating and updating educational resources that make complex benefit and work incentive topics easier for clients and families to understand.
  • Preparing quarterly reporting that highlights program outcomes, service activity, and progress toward contract deliverables.
  • Delivering training and technical guidance to community members and human service professionals regarding disability-related work incentives and referral pathways.
  • Referring beneficiaries to suitable employment networks and coordinating with specialized work incentive contacts and field office resources as needed.
