Heartland Hospice seeks a compassionate MSW Social Worker to support patients with life-limiting illness and their families. In this role, you'll complete psychosocial assessments, develop care plans, and provide counseling, crisis intervention, and resource linkage in homes and facilities. Collaborating with an interdisciplinary hospice team, you'll advocate for patient wishes, assist with advance care planning, and coordinate community services. This mission-driven position offers opportunities to grow clinically, participate in team meetings and bereavement support, and uphold dignity, respect, and comfort at end of life.

Responsibilities

  • Conduct psychosocial assessments for hospice patients and families
  • Develop and implement individualized psychosocial care plans
  • Provide counseling, emotional support, and crisis intervention
  • Educate and advocate for patient and family needs and goals of care
  • Facilitate advance care planning and end-of-life decision support
  • Coordinate community resources and support services
  • Participate in interdisciplinary team meetings and care conferences
  • Document services and assessments in accordance with regulatory standards
  • Support bereavement and grief resources for families
  • Maintain compliance with hospice, state, and federal regulations
