Requirements
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- CHPN certification (Preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience

Roles & Responsibilities
- Assess patients in home health and/or hospice to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families regarding care, medications, and home safety, employing appropriate teaching methods.
- Supervise and guide Home Health Aides and other support staff to ensure high-quality care.
- Monitor assigned cases for compliance with third-party payer requirements.
- Communicate effectively with patients, families, and the healthcare team to coordinate care.

Compliance / Notes
- Must demonstrate commitment, professional growth, and maintain current licensure certifications.
- Ability to work a flexible schedule and travel locally.
- Maintain confidentiality in all patient-related matters.
