MDS Coordinator Must be RN/LPN
Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois is seeking a dedicated and experienced MDS Coordinator RN/LPN to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents. This is a full-time, day position that requires strong assessment, documentation, and leadership skills.
Aboutthe Role
As an MDS Coordinator, you will be responsible for completing and submitting accurate and timely MDS assessments, reconciling documentation with orders and therapy, and managing PDPM/RUG coding, care plans, quality audits, and staff training. You will also support surveys and QA/QAPI activities, ensuring compliance with regulatory requirements.
Responsibilities- Complete and submit MDS assessments (admission/quarterly/annual/SC/discharge) on time
- Conduct resident assessments and reconcile documentation with orders and therapy
- Manage PDPM/RUG coding, care plans, quality audits, and staff training
- Support surveys and QA/QAPI activities
- Collaborate with interdisciplinary team members to ensure high-quality care
- Active Illinois RN or LPN license
- 2+ years of long-term care experience; MDS experience preferred
- Familiarity with Point Click Care or similar EHR; strong assessment/documentation skills
