As a Memory Care LPN, you play a vital role in delivering compassionate, person‑centered care to residents living with Alzheimer’s or other forms of dementia. You’ll provide clinical support, ensure safe medication management, and foster a calm, engaging environment where every resident feels valued and respected.

• Provide compassionate nursing care to residents within our Memory Care neighborhood
• Administer medications and treatments in accordance with physician orders and state regulations
• Conduct health assessments and contribute to individualized care plans
• Monitor changes in residents’ conditions and communicate promptly with leadership and families
• Support and mentor caregivers to ensure consistent, quality care delivery
• Maintain accurate documentation and uphold safety and infection control standards
• Promote a positive, team‑oriented culture aligned with Charter Senior Living’s mission

License: Active LPN license in the State of Illinois (required).
Experience: Prior memory care or senior living nursing experience strongly preferred.
Skills: Strong clinical, communication, and leadership skills.

Schedule: Availability for 2nd and/or 3rd shifts (Part‑Time).
