Life’sWork of Western PA is seeking a compassionate Case Manager to provide in-home case management services for children, youth, and families. The role partners with referral sources, families, providers, and county agencies to support positive outcomes and connect clients with community resources.
The Case Manager conducts regular visits, develops service plans, coordinates care, documents thoroughly, and maintains compliance with agency and county requirements.
