Heritage Senior Living is seeking a compassionate, organized, and dependable Wellness Coordinator to help lead our resident care team and ensure residents receive consistent, high-quality, individualized care.

Reporting to the Director of Nursing (DON) or designee, the Wellness Coordinator provides hands-on leadership and support to CNA/Resident Assistants while coordinating daily resident care needs, wellness activities, staffing, documentation, and communication.

This is an ideal opportunity for an experienced caregiver, Resident Assistant, CNA, or care team leader who is ready to take on greater responsibility and make an impact on both resident care and team performance.

What You’ll Do Coordinate Daily Resident Care Complete morning and afternoon rounds throughout assigned areas.

Review shift updates and the 24-hour communication report to maintain continuity of care.

Speak with care team members at the beginning of the shift to identify resident needs, changes, and priorities.

Follow up throughout the day to ensure resident needs and assigned care responsibilities are completed.

Read and understand resident Individualized Service Plans (ISPs) and help ensure services are provided according to each resident's plan.

Contribute observations regarding resident needs, preferences, and changes to the service-planning process.

Complete scheduled wellness checks and respond promptly to resident needs.

Promptly communicate significant changes or concerns to the LPN, DON, or other appropriate clinical team member.

Lead & Support the Care Team Provide day-to-day leadership and direction to CNA/Resident Assistants.

Confirm assigned employees are present and appropriately deployed within their assigned areas.

Coordinate and adjust assignments and staffing based on resident and community needs.

Train, mentor, coach, and support CNA/Resident Assistants.

Participate in employee coaching and corrective-action processes as appropriate.

Assist with new Resident Assistant orientation and required skills checks.

Monitor employee documentation and assignment completion.

Help ensure employees follow resident ISPs, safety procedures, infection-control practices, and community standards.

Lead by example and provide hands-on resident care when needed.

Coordinate Vitals, Weights & Wellness Monitoring Coordinate and delegate assigned resident weights and vital signs to appropriately trained CNA/Resident Assistants.

Follow up to ensure assigned weights and vital signs are completed and documented accurately.

Monitor completion of required vital signs associated with antibiotics, falls, or other identified resident needs.

Communicate abnormal findings or concerns promptly to the appropriate licensed clinical team member.

Help ensure clinical follow-up responsibilities are completed as directed.

Coordinate Showers, Baths & Laundry Maintain and coordinate resident shower and bathing schedules according to ISPs and resident preferences.

Follow up with employees to ensure scheduled showers and baths are completed and documented.

Help coordinate resident laundry responsibilities and follow up on completion.

Address missed or incomplete services promptly to help ensure residents receive consistent care.

Support Hospice & Outside Providers Maintain current information regarding residents receiving hospice services.

Coordinate hospice schedules, including nursing/aide visits and showers provided by hospice personnel.

Provide appropriate resident information, such as weights and bowel-movement documentation, to hospice providers as authorized and required.

Follow up regarding durable medical equipment (DME) provided through hospice, including wheelchairs, cushions, beds, and mechanical lifts.

Communicate with hospice and other outside providers to support coordinated resident care.

Support Diet & Nutrition Communication Help ensure resident ISPs accurately reflect current diet instructions.

Communicate diet changes promptly to culinary staff and appropriate care team members.

Follow up on identified dietary needs and concerns.

Communicate resident changes affecting diet or nutrition to the appropriate licensed clinical team member.

Support Life Safety & Quality Assurance Assist with routine quality and life-safety monitoring.

Complete or coordinate checks related to:

Elopement risks and applicable safety measures.

Durable medical equipment.
