Position Summary
As an LPN in our Primary Care practice, you will play a vital role in delivering preventive care, managing chronic conditions, and ensuring a seamless patient experience. Working within the Nursing Professional Practice Framework, you will provide quality, efficient, and compassionate care to patients across the lifespan.

Key Responsibilities
* Partner with providers and care teams to support preventive care and chronic disease management
* Room patients, obtain vitals, and document health information accurately in the EMR
* Administer medications, vaccines, and treatments per provider orders
* Educate patients on health conditions, medications, and follow-up care
* Coordinate care, including referrals, labs, and follow-up appointments
* Respond to patient inquiries and support a positive, patient-centered experience
What We're Looking For
Required:
* Active LPN license in the state of Connecticut
* Strong clinical and organizational skills
* Excellent communication and patient engagement abilities
Preferred:
* Primary care, ambulatory, or outpatient experience
* Familiarity with EMR systems
* Experience with care coordination or population health initiatives
