Position Summary

As an LPN in our Primary Care practice, you will play a vital role in delivering preventive care, managing chronic conditions, and ensuring a seamless patient experience. Working within the Nursing Professional Practice Framework, you will provide quality, efficient, and compassionate care to patients across the lifespan.

Key Responsibilities
  • Partner with providers and care teams to support preventive care and chronic disease management
  • Room patients, obtain vitals, and document health information accurately in the EMR
  • Administer medications, vaccines, and treatments per provider orders
  • Educate patients on health conditions, medications, and follow-up care
  • Coordinate care, including referrals, labs, and follow-up appointments
  • Respond to patient inquiries and support a positive, patient-centered experience
Qualifications

What We’re Looking For

Required
  • Active LPN license in the state of Connecticut
  • Strong clinical and organizational skills
  • Excellent communication and patient engagement abilities
Preferred
  • Primary care, ambulatory, or outpatient experience
  • Familiarity with EMR systems
  • Experience with care coordination or population health initiatives
