As an LPN in our Primary Care practice, you will play a vital role in delivering preventive care, managing chronic conditions, and ensuring a seamless patient experience. Working within the Nursing Professional Practice Framework, you will provide quality, efficient, and compassionate care to patients across the lifespan.
Key Responsibilities- Partner with providers and care teams to support preventive care and chronic disease management
- Room patients, obtain vitals, and document health information accurately in the EMR
- Administer medications, vaccines, and treatments per provider orders
- Educate patients on health conditions, medications, and follow-up care
- Coordinate care, including referrals, labs, and follow-up appointments
- Respond to patient inquiries and support a positive, patient-centered experience
What We’re Looking For
Required- Active LPN license in the state of Connecticut
- Strong clinical and organizational skills
- Excellent communication and patient engagement abilities
- Primary care, ambulatory, or outpatient experience
- Familiarity with EMR systems
- Experience with care coordination or population health initiatives
