Providence Care is seeking a Licensed Masters Level Social Worker (LMSW) to provide full-time hospice social work services to patients in their homes, assisted living facilities, or skilled nursing facilities across several counties in Georgia.
The LMSW will supervise and evaluate social services in accordance with agency policies, coordinate care, counsel families, participate in discharge planning, and connect patients with community resources while traveling as needed.
