RN Acute Care Navigator: Care Coordination & Discharge

Pro Medica Monroe Regional Hospital is seeking an RN Acute Care Navigator to oversee patient care, coordinate transitions, and optimize length of stay. You will work with interdisciplinary teams to assess needs and develop discharge plans, while educating patients on medications and post-discharge care.
