- Assess patient needs and collect data on physical, psychological, social, and spiritual factors that influence health, and incorporate that data into the plan of care.
- Conduct initial evaluation and re-evaluate nursing needs during each visit; follow up visits as needed by patient, family, and facility.
- Communicate significant findings and changes in health condition to the Clinical Supervisor, physician, facility, and other care team members.
- Revise the plan of care in response to identified issues and notify relevant team members.
- Counsel, instruct, and include patient, facility, and family in following the Interdisciplinary Plan of Care.
- Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
- Collaborate with Medical Director, attending physician, branch leadership, and IDG.
- Perform other duties or projects as assigned.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse (RN).
- 1 year of experience in a clinical care setting.
- Experience in hospice or home health environment preferred.
- Nursing skills as defined by accepted nursing standards.
- Ability to handle multiple priorities, documentation requirements, and deadlines.
- Able and willing to travel up to 50% as needed to support business and patient needs.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license and insurance and reliable transportation to perform job tasks.
You will report to the Clinical Team Manager.
